Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to provide high-quality patient care in our skilled nursing and long-term care units. Functions include medication administration, IVs where applicable, vital signs, and patient education.

The ideal candidate holds a current LPN license and will collaborate closely with physicians, NPs, and the healthcare team, including supervising CNAs. We offer daily pay options and a $3,000 sign-on bonus.

Qualifications

• Current LPN license.
• Experience in skilled nursing/long-term care is a plus.

Responsibilities

• Provide care for residents in SNF and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with outcome monitoring.
• Educate residents and families.
• Communicate with physicians, NPs, families and other healthcare professionals.
