Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced Licensed Practical Nurse (LPN) to join our skilled nursing and long-term care units. You will provide resident care, administer medications (including IVs where permitted), monitor vital signs, and educate residents and families.

Ideal candidates hold an active LPN license, collaborate with physicians and healthcare staff, and help supervise CNAs. We offer a $3,000 sign-on bonus and daily pay options.

Qualifications

• Active LPN license required.

Responsibilities

• Provide care for SNF and Long Term Care unit residents.
• Medication administration, including IVs where applicable.
• Take and record vital signs and monitor treatment outcomes.
• Educate residents and families about care plans and treatments.
• Communicate with physicians, NPs, families and other health professionals; supervise CNAs.
