The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to care for residents in our skilled and long-term care units. Responsibilities include medication administration, vital signs, and patient education, with a focus on compassionate, high-quality care.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

Licensed Practical Nurse (LPN) QUALIFICATIONS/REQUIREMENTS:

• Current License-LPN

Salary: Up to $42 an hour
