Job summary
The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide direct care to residents in the Skilled and Long Term Care Units, administer medications (including IVs) and monitor patient outcomes, while educating residents and families and coordinating with the medical team.

Qualifications
- Active LPN license in New York.
- Experience in SNF/long-term care is a plus.
- Ability to supervise CNAs and coordinate with physicians and staff.
Responsibilities
- Provide care for residents in the Skilled Unit and Long Term Care Unit.
- Administer medications, including IVs.
- Take and record vital signs.
- Provide resident care and treatments with monitoring of outcomes.
- Educate residents and families.
- Communicate with physicians, NPs, families and other health care professionals.
- Supervise CNAs.
