Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver compassionate nursing care to pediatric and/or adult patients in their homes, across New York.

The role emphasizes applying the nursing process, assessing patients, and coordinating care with the health team. Qualified candidates hold an active LPN/LVN license in NY and possess current BLS certification; TB/Chest X-ray clearance is required, and1 year of nursing experience is preferred.

Qualifications

• Current LPN/LVN license in the state of practice.
• TB test or Chest X-ray current.
• Current BLS certification.
• One year of professional nursing experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide skilled nursing care/services per prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and interpret data from health care members and document nursing diagnoses.

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
