Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to pediatric and/or adult patients in their homes across New York.

The role focuses on delivering patient-centered care using the nursing process and prescribed orders. Ideal candidates hold active LPN/LVN licensure in NY, a current TB or chest X-ray, and a valid BLS card; one year of nursing experience is preferred.

Qualifications

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
