Job summary

The Lakes Home Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and/or adult patients.

This role requires applying the nursing process, delivering skilled nursing services per orders, and monitoring patient status in the home setting. Qualified candidates hold an LPN/LVN license in the practicing state, have a current BLS card, and prefer candidates with at least one year of nursing experience.

Qualifications

• LPN/LVN license in the state where practice is required.
• TB test or Chest X-ray current/complete.
• Current BLS card.
• One year of professional nursing experience preferred.

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• On each assignment, provides skilled nursing care/services in accordance with prescribed orders.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data and information from health care members and documents Nursing diagnoses.

• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred
