Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset in New York is seeking a Licensed Practical Nurse (LPN) to join our skilled unit and long‑term care team. You will administer medications, monitor vital signs and provide resident care with education and documentation.

Collaboration with physicians, NPs, families and CNAs is required. A current LPN license is mandatory, and we offer a $3,000 sign‑on bonus, daily pay, and a robust benefits package including 1199 union benefits and

Qualifications

• Current LPN license in good standing.
• Experience in SNF/long-term care preferred.
• Ability to administer medications, including IVs if required.

Responsibilities

• Provide care for residents in the Skilled and Long Term Care Units.
• Administer medications and monitor for outcomes.
• Take and record vital signs and provide resident education.
• Coordinate with physicians, NPs, families and other health care professionals.
• Supervise CNAs and support care plan implementation.
