Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a licensed practical nurse (LPN) to join our skilled nursing team in Port Jefferson, NY. The role includes medication administration, including IVs, taking vital signs, and delivering resident care and treatments with ongoing outcome monitoring.

You will also provide resident and family education and communicate with physicians, NPs, families, and other health care professionals while supervising CNAs.

Qualifications

• Current LPN license required.
