Job summary

Angels Care Home Health is seeking an LPN to provide skilled nursing care in patients' homes under supervision of a registered nurse. You will coordinate care with the interdisciplinary team and families to ensure optimal outcomes.

Qualified candidates have an accredited LPN program diploma, an active LPN license, and a valid driver’s license. CPR and Hepatitis profile must be current; home health experience is preferred. We offer comprehensive benefits and opportunities for growth.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing program.
• Current state LPN license.
• Current State’s Driver’s License.
• Two years’ LPN experience; home health preferred.
• CPR and Hepatitis profile up to date.

• Coordinate care with interdisciplinary team, patient/family, and referring agency.
