Job summary

Angels Care Home Health is seeking a licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. You will coordinate with the care team, patients and families, and referring agencies to ensure comprehensive care.

Qualifications include graduation from an accredited LPN program, active LPN license, CPR certification and Hepatitis profile, and at least two years of LPN experience, preferably in home health. Driver's license required.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license.
• Two years of LPN experience; home health preferred.
• Proof of CPR certification and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Deliver care to clients of all ages in their place of residence.
