Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients’ residences under the supervision of a registered nurse. The position involves coordinating care with the interdisciplinary team, patient and family, and referring agencies.

The ideal candidate holds an active LPN license, a valid driver's license, CPR certification, and Hepatitis profile, with two years of LPN experience, preferably in home health.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
