Job summary

Angels Care Home Health is seeking a licensed practical nurse (LPN) to provide skilled nursing care in clients' homes across Cottonwood, AZ. The role operates under supervision of a registered nurse and involves coordinating care with families and the broader care team.

Qualified candidates hold an active LPN license in good standing, a valid driver’s license, CPR certification, and hepatitis profile. Home health experience is preferred.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Current State’s Driver’s License.
• Two years’ LPN experience; home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care in clients' homes under supervision of a registered nurse.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
