Job summary

Angels Care Home Health in Smithville, OK is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. You will coordinate with the interdisciplinary team, patients and families to ensure quality, compassionate care delivered in the patients' residence.

Eligible candidates have an accredited practical nursing program diploma, an active LPN license, and a valid driver's license.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license.
• Valid driver's license.
• Minimum two years' LPN experience; home health preferred.
• CPR certification and Hepatitis profile up to date.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
• Provide care in clients' homes and maintain accurate documentation.
