Job summary

Angels Care Home Health seeks a licensed practical nurse (LPN) to deliver skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patients, families and referring agencies to ensure high-quality home health services.

Applicants should be a graduate of an accredited practical nursing program with an active LPN license, valid driver's license, CPR certification, and at least 2 years of home health experience.

Qualifications

• Graduate of an accredited school of practical nursing (LPN).
• Current state license as an LPN.
• Current state's driver's license.
• Two years’ experience as an LPN, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision in clients' home environments.
• Coordinate care with interdisciplinary team, patient/family and referring agency.
