Job summary

Angels Care Home Health in Anthem, AZ seeks a Licensed Practical Nurse (LPN) to administer skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, the patient and their family, and referring agencies.

Required qualifications include graduating from an accredited practical nursing program, possessing an LPN license, a valid driver’s license, and at least two years of LPN experience (home health preferred).

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
