Job summary

Angels Care Home Health is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes throughout Milton, FL. Under RN supervision, you will coordinate with the care team, patient, family, and referring agency to ensure optimal outcomes.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
