Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to administer skilled nursing care in clients' homes under RN supervision. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure quality care.

Requirements include graduation from an accredited practical/vocational nursing program, an active LPN license, a valid driver’s license, and CPR/Hepatitis verification.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence.
• Coordinates care with the interdisciplinary team, patient/family, and referring agency.
