Job summary

Premise Health is seeking a Full-Time Licensed Practical Nurse to provide direct patient care at the Viatris Family Health Center in St. Albans, VT. The role supports client employees and dependents, with hours Mon/Wed/Thu 8:00a-4:30p and Tue/Fri 6:00a-2:30p.

The position offers a comprehensive benefits package including medical, dental, vision, 401k, paid time off and holidays, CME and tuition reimbursement, plus a $2,000 sign-on bonus.

Qualifications

• Graduate of National League for Nursing accredited program.
• Currently Licensed as a Practical Nurse in state of practice required.
• Current certification with hands-on training in Basic Life Support (BLS) required.

Responsibilities

• Provide direct patient care and collaborate with healthcare providers.
• Perform and document medical tasks including medication administration, wound care, specimen collection per provider orders.
• Maintain medical equipment and infection control procedures.

As a Full Time, Licensed Practical Nurse, you'll provide care to client employees and their dependents at theViatris Family Health Center, powered by Premise Health,located in St. Albans, VT.

The scheduled hours are Monday/Wednesday/Thursday 8:00a - 4:30p. Tuesday/Friday 6:00a - 2:30p.

What You’ll Do

• Provides direct patient care and collaboration under the supervision of healthcare providers, and within scope of practice.
• Performs and documents medical tasks to include medication administration wound care, observes reactions to treatments, and specimen collection such as urine, blood, and sputum based on provider orders.
• Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures.
• Maintains patient health records to ensure accurate and up-to-date records.
• Performs other duties as assigned.

What You’ll Bring

• Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school.
• Currently Licensed as a Practica Nurse in state of practice required.
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required.

Premise provides its reasonable and genuinely expected range of compensation for this job of $21.60 - $27.00 per hour. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
