Job summary

Angels Care Home Health in Missouri is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care in clients' homes under a registered nurse supervisor. You will coordinate with the interdisciplinary team, patients and families to support their care plans.

The ideal candidate is a graduate of an accredited practical nursing program, holds an LPN license, has a valid driver's license, and at least two years of LPN/home health experience. CPR and Hepatitis profiling are required.

Qualifications

• Graduate of an accredited school of practical nursing.
• Current state license as an LPN.
• Two years’ LPN experience; home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
