Job summary

Angels Care Home Health is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patients and families to deliver compassionate, high-quality services.

Qualifications include graduation from an accredited LPN program, an active LPN license, and a valid driver's license. Experience in home health and CPR certification preferred.

Qualifications

• Graduate of an accredited school of practical nursing or vocational nursing.
• Active LPN license in the state.
• Valid driver's license in state of residence.
• Two years’ experience as an LPN, home health preferred.
• Proof of CPR certification.

Responsibilities

• Administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
