Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under the supervision of an RN. You will coordinate care with the interdisciplinary team, patients and families, and referring agencies to ensure comprehensive, compassionate service.

The ideal candidate has an accredited practical nursing diploma, an active LPN license, a valid driver’s license, CPR, and hepatitis profile. Home health experience is preferred.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision in patients’ residences.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
