Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to administer skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patient and family, and referring agencies to ensure high-quality outcomes.

The ideal candidate holds an accredited LPN diploma, an active LPN license, and a valid driver’s license with CPR and Hepatitis profile up to date. Two years’ home health experience is preferred.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse.
• Coordinate care with the interdisciplinary team and family.
• Collaborate with referring agency for patient needs and plan of care.
• Document patient care activities and communicate changes to the team.
