Job summary

Angels Care Home Health is actively seeking an LPN to provide skilled nursing services in clients' homes. You will work under the supervision of a registered nurse, coordinating with the family and the interdisciplinary team to ensure quality care in the patient’s residence.

The role requires an active LPN license, a valid driver’s license, CPR and Hepatitis verification, and at least two years of LPN experience with home health preferred.

Qualifications

• Graduate of accredited practical/vocational nursing program.
• Current state license as an LPN.
• Driver's license in the state of practice.
• Two years' LPN experience, home health preferred.
• CPR certification and Hepatitis profile required.

Responsibilities

• Administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with interdisciplinary team, patient/family, and referring agency.
