Job summary

Ascension Living in Avilla, IN, is seeking a dedicated Licensed Practical Nurse for full-time day shifts at Sacred Heart Village. You will deliver hands-on nursing care, administer medications, perform treatments, and conduct routine assessments to support residents in a long-term care setting.

You will monitor residents’ conditions, report changes to RNs, and build trusting relationships with residents and families.

Qualifications

• BLS Provider within 1 month (30 days) of hire date or job transfer date required.
• Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date required.

Responsibilities

• Delivers hands-on nursing care, including medication administration, treatments, and routine assessments.
• Supports RNs by monitoring resident conditions and promptly reporting changes.
• Builds consistent, trusting relationships with residents and families in a long-term care setting.
• Detail-oriented, compassionate, and committed to resident safety and comfort.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Licensed Practical/Vocational Nurse obtained prior to hire date or job transfer date required.

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
