Job summary

Elevance Health is seeking a Behavioral Health Case Manager I to perform telephonic and home-visit case management within the licensed scope for members with behavioral health and substance use needs.

Qualifications

• Requires MA/MS in social work, counseling, or related behavioral health field or nursing degree with relevant experience.
• Active unrestricted licenses such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW or Clinical Psychologist where applicable.
• Experience with complex psychiatric and substance abuse treatment preferred.

Responsibilities

• Uses screening criteria knowledge and clinical judgment to assess member needs.
• Conducts assessments and develops care plans to address identified objectives.
• Monitors and modifies care plans based on progress and outcomes.
• Coordinates with internal/external resources and providers to meet needs.
