Job summary

CVS Health is seeking a clinical case manager to apply advanced clinical judgement in coordinating member physical and behavioral health care, including psychosocial wraparound services.

You will plan care, direct provider collaboration, and leverage available resources to achieve optimal, cost-effective outcomes while meeting performance metrics. The role involves travel within a 125‑mile radius and a 40-hour work week with some evenings.

Qualifications

• Current/Active clinical licensure (LCSW, LPC, LMFT)
• Case management experience
• Strong customer service orientation and problem solving
• Excellent communication/telephonic skills
• Excellent motivational interviewing skills and ability to build rapport telephonically
• Excellence in documentation and regulatory compliance
• Highly organized and self-driven
• Ability to travel within 125 miles round trip for 1 day in the field
• Variable work schedule with ability to work 2 days weekly until 9pm local time
