Job summary

CVS Health is seeking a licensed clinician to coordinate physical and behavioral health care for members. You will assess needs, plan care, direct provider collaboration, and coordinate psychosocial wraparound services to deliver optimal, cost-effective outcomes.

This full-time role requires active LCSW/LPC/LMFT licensure, strong communication and motivational interviewing skills, and willingness to travel up to 125 miles for in-field visits.

Qualifications

• Current active licensure as LCSW/LPC/LMFT required.
• Case management experience and coordination with providers.
• Strong customer service orientation and problem solving.
• Excellent telephonic communication and documentation practices.
• Motivational interviewing skills to build rapport telephonically.
• High organizational skills and regulatory-compliant documentation.
• Ability to work independently and manage time effectively.
• Meet performance metrics and adapt to schedule.
• Proficient with technology and field travel up to 125 miles.
• Flexibility to work two evenings per week when needed.
