Job summary

CVS Health is seeking a licensed clinician to apply advanced clinical judgement, coordinate provider collaboration, and manage psychosocial wraparound services to optimize member health outcomes.

The role requires active licensure (LCSW/LPC/LMFT), extensive case management, strong communication and motivational interviewing skills, and meticulous documentation. Travel up to 125 miles for field visits is required; 40-hour full-time schedule with some evenings.

Qualifications

• Current/Active Clinical Licensure (LCSW, LPC, LMFT).
• Case Management experience.
• Strong customer service orientation and problem solving.
• Excellent Communication/Telephonic skills.
• Excellent motivational interviewing skills/ability to build rapport and trust telephonically.
• Excellence in documentation and respecting compliance/regulatory standards.
• Highly organized, self-driven with no direct supervision.
• Time management skills.
• Ability to travel within 125 miles round trip for 1 day in the field.
• Variable work schedule with ability to work 2 days weekly until 9pm local time.

Responsibilities

• Applies advanced clinical judgement to facilitate appropriate member physical health and behavioral healthcare.
• Implements strategy to assess and plan care, direct provider collaboration, and coordinate psychosocial wraparound services.
• Instructs the application of available resources to produce optimal, cost-effective outcomes.
