Job summary

CVS Health is seeking a clinically skilled professional to assess and coordinate member health and behavioral care, using advanced clinical judgment to promote optimal outcomes. The position requires 3–5 years of post-master's clinical practice, an active Behavioral Health license, and strong discharge planning experience.

Qualifications

• Master's degree in Behavioral/Mental Health or related field.
• Active Behavioral Health clinical license in the state where you work.
• 3–5 years direct clinical practice post-master's degree.
• Crisis intervention, case management and discharge planning experience preferred.

Responsibilities

• Assess referred member needs/eligibility and determine care approach.
• Apply clinical judgment to risk factors and care plans.
• Perform crisis intervention and refer to clinicians for assessment.
• Provide crisis follow-up to ensure appropriate treatment.
• Interpret guidelines and coordinate benefits with programs/services.
• Collaborate with supervisors/MDs to overcome barriers and optimize outcomes.
• Identify and escalate quality of care issues through established channels.
• Engage with medical/behavioral health professionals to influence care.
• Use motivational interviewing to improve member engagement and health behaviors.
• Coach and empower members to participate in healthcare decisions.
• Analyze data to identify member needs and plan care with the team.
• Develop and monitor plans of care with the member and care team.
