Job summary

CVS Health in the United States is seeking an experienced clinician to apply advanced clinical judgment to coordinate member physical and behavioral health care. The role directs provider collaboration and coordinates psychosocial wraparound services to achieve optimal outcomes.

The position requires active licensure (LCSW, LPC, LMFT), case management experience, strong telephonic communication, and the ability to travel up to 125 miles for a 1-day field visit.

Qualifications

• Current/Active Clinical Licensure (LCSW, LPC, LMFT).
• Case management experience.
• Strong customer service orientation and problem solving.
• Excellent communication/telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport telephonically.
• Excellence in documentation and respecting compliance/regulatory standards.
• Highly organized and self-driven without direct supervision.
• Manage time effectively.
• Achieve performance metrics.
• Technology proficiency.
• Ability to travel within 125 miles round trip for 1 day in the field.
• Variable work schedule with ability to work 2 days weekly until 9pm local time.
• CCM Certification (Preferred).
• Experience in Managed Care Organization (Preferred).

Responsibilities

• Applies advanced clinical judgement to facilitate member physical and behavioral health care.
• Implements strategy to assess and plan care, direct provider collaboration, and coordinate psychosocial wraparound services.
• Instructs the application of available resources to produce optimal, cost-effective outcomes.
