The Lead Case Manager develops ISPs, coordinates care with service providers, and ensures accurate documentation.

Qualifications

• Bachelor’s degree in a field related to social services or public health.
• Minimum 3 years of direct service experience with low-income individuals, behavioral health, and homelessness.
• Supervisory or project management experience required.
• Strong understanding of harm reduction and case management practices.
• Experience with documenting case notes, ISP development, and collaboration with partners.

Responsabilités

• Oversee the case management team, ensure quality service delivery and compliant documentation.
• Coordinate on-call scheduling and manage caseloads effectively.
• Develop and manage Individual Service Plans (ISPs) with residents to address goals and barriers.
• Lead outreach activities to connect residents with needed services.
• Maintain collaboration with community partners and oversee referrals and care coordination.
• Provide intensive case management support and ensure continuity of care across providers.
• Maintain confidentiality and professional standards in all records and communications.
• Attend required meetings and contribute to program improvements.
