Job summary

Heluna Health, through Community Health Project Los Angeles (CHPLA), seeks a Case Manager for the LEAD program to provide field-based intensive case management to participants in Los Angeles. The role focuses on harm reduction, housing stability, and coordinated care, with data entry into CHAMP within 24–48 hours of contact.

Responsibilities include assessing needs, linking to services, and supporting participants with housing, health care, and transportation, under Housing First principles.

Qualifications

• High school diploma or equivalent required; Bachelor’s degree preferred.
• Experience linking people to housing, health care, entitlements, and treatment preferred.
• Experience or fieldwork serving people who use drugs, people at risk for overdose, and/or vulnerable populations.

Responsibilities

• Provide care from a truly harm reduction, nonjudgmental, participant-centered, and low-threshold approach.
• Enroll participants, assess needs, and link to services and care.
• Assess immediate needs at intake including medical care, shelter, food, clothing, and harm reduction services/supplies.
• Develop and implement initial assessments and care plans addressing client needs and goals.
• Update assessments and care plans quarterly to reflect progress and emerging needs.
• Maintain collaborative relationships with community partners and service providers.
• Educate participants on CHPLA services, Empowerment Program, SSP and MAT services, and related resources.
• Enter data into CHAMP system within 24-48 hours of contact; submit incident reports within 24 hours.
• Adhere to contract requirements for charting, assessment, and documentation.
• Provide intensive case management per harm reduction and Housing First principles.
• Attend regular meetings and contribute to team operations.
• Develop and maintain client files per contract requirements.
• Travel as needed to support participant care.
