Job summary

The People Concern in Santa Monica is seeking a Lead Case Manager for the LEAD program to support chronically homeless individuals referred from LAPD Pacific division. The role involves outreach, engagement, aftercare planning, and connecting clients to services to prevent entry into the jail and criminal justice system.

The Senior Case Manager will demonstrate wellness, recovery, and coping skills while coordinating with the LEAD ODR team and external agencies to ensure high-quality service

Qualifications

• High school diploma or GED required.
• Bachelor's degree preferred; experience with homeless populations is a plus.
• Valid CA driver's license and ability to transport clients in personal vehicle.
• Knowledge of LA County agencies serving the homeless.

Responsibilities

• Identify and engage LEAD program clients living on the streets and vulnerable due to medical, psychiatric, or addiction needs.
• Collaborate with team to remove barriers to permanent housing (ID, legal aid, benefits, life skills).
• Provide case management and aftercare/follow-up for housed clients.
• Maintain accurate electronic records and participate in staff meetings.
