Qualifications

• Bachelor's degree or 4+ years direct case management experience.
• Strong interpersonal and written communication skills.
• Familiarity with trauma-informed care, Harm Reduction, Housing First principles.

Responsibilities

• Assess clients’ needs and goals and develop service plans.
• Coordinate referrals to housing, health, and social services.
• Advocate for client rights and liaise with community agencies.
• Maintain confidential, accurate case records and progress notes.
• Provide supportive counseling and life-skills coaching to clients.
• Collaborate with other case managers to ensure holistic care.
• Contribute to program evaluation and service improvements.
