Qualifications

• Master’s or Bachelor’s in social work, psychology, counseling, or related field; 5+ years of management experience.
• Certification in case management (CCM/ACM) or CADC or MHFA preferred.
• Experience with Homeless Response System and harm-reduction approaches.
• Proficiency with case management software and Microsoft Office.

Responsibilities

• Manage a caseload of complex cases and provide direct support.
• Develop and monitor individualized care plans every 90 days with clients.
• Coordinate access to healthcare, housing, education, employment, and social services.
• Conduct in-person client check-ins and maintain calendars.
• Ensure documentation and notes are entered within 48 hours using PIRP format.
• Participate in supervision, meetings, and stakeholder collaborations.
