Job summary

Bayview Hunters Point Foundation for Community Imp is seeking a Case Manager II to provide comprehensive support to individuals and families experiencing homelessness in San Francisco. You will assess needs, develop service plans, and connect clients to housing, healthcare, and social resources, while advocating for rights and coordinating with partners.

You will monitor progress, maintain confidential records, offer counseling, and lead life skills sessions.

Qualifications

Bachelor's degree in social services, psychology or a related field is required or 4+ years direct case management experience.
Experience with vulnerable populations and homelessness is advantageous.
Trauma-informed care and client-centered approaches are valuable.

Responsibilities

Conduct comprehensive assessments to identify needs, strengths and goals.
Develop individualized case plans addressing housing, employment, healthcare and social support.
Refer clients to housing programs, mental health services, substance abuse treatment, job training and legal aid.
Advocate for clients within shelters and with social service agencies.
Provide crisis intervention and document progress in confidential records.
Offer supportive counseling and life skills training to build coping and independence.
Collaborate with other case managers and external service providers for holistic care.
Assist in program evaluation and stay updated on best practices in homelessness services.
