Job summary

Health Care for the Homeless Inc in Baltimore seeks a Lead Case Manager to supervise case managers, provide coaching, and support high-quality service delivery. You will participate in team meetings and promote coordinated, client-centered care.

Responsibilities include hands-on case management, treatment planning, referrals, and ensuring comprehensive documentation in the EHR. Bachelor’s degree required; supervisory experience preferred; CAC certification within six months.

Qualifications

• Bachelor’s degree required, BSW preferred.
• Obtain Certified Application Counselor certification within six months of hire.
• Experience in case management with vulnerable populations recommended.

Responsibilities

• Provide day-to-day supervision, coaching, and support to case managers.
• Deliver case management services including assessments, treatment planning, referrals, and warm engagement.
• Ensure timely, accurate documentation in the Electronic Health Record and coordinate resource referrals.
• Collaborate with multidisciplinary teams and participate in performance improvement activities.
