Job summary

People-Encouraging-People seeks a Case Manager in Baltimore, MD to provide case coordination for unhoused individuals and those with behavioral health concerns, including SUD. You will coordinate between the Agency, community, and home settings to ensure integrated care.

Responsibilities include intake, needs assessment, care coordination, and advocacy, with emphasis on documentation, crisis intervention, and collaboration with shelters and housing resources.

Qualifications

• 5+ years of experience providing case management to vulnerable populations.
• High School Diploma or equivalent; bachelor’s or associate degree in social services preferred.
• Valid Driver’s License and use of personal vehicle.
• Experience with case management databases; HMIS preferred.
• Excellent communication, problem solving, and organizational skills.

Responsibilities

• Screen clients upon admission to the Homeless Services program and complete intake packets.
• Meet clients to evaluate needs and create individualized case plans.
• Coordinate client care and advocate with community agencies; refer to services as required.
• Monitor progress, update treatment plans, and ensure goals are met.
• Maintain confidential documentation per policies and procedures.
• Educate clients and families on accessing resources and recourses.
• Provide advocacy and crisis intervention when necessary.
• Participate in community meetings, client care conferences, and related programs.
• Visit agencies, shelters, and hospitals to assess needs and housing eligibility.
