Job summary

People Encouraging People in Baltimore seeks a dedicated professional to assess clients’ needs and guide them toward goals. You will complete annual assessments, reassessments and individualized service plans, and help link clients to medical care, housing, entitlements, and legal aid.

Qualifications

A minimum of an Associate's degree in mental health and 1 year of mental health experience.
Excellent organizational skills and ability to prioritize tasks.
Position requires great flexibility due to not having a set schedule or agenda.

Responsibilities

Establish and maintain client medical records in keeping with COMAR regulations and Agency standards.
Complete annual assessments, reassessments and individualized service plans.
Help ensure a smooth transition into client communities.
Establish linkages to needed resources including medical care, housing, entitlements, and legal assistance.
liaison with family and representatives from service agencies in the community.
Provide referral, intake and orientation processes with new clients.
Accompany clients to appointments as needed.
