Job summary

Primary Health Care (PHC) in Des Moines, IA is seeking a Case Manager Team Lead for Homeless Support Services. You will lead assigned case management staff, provide client advocacy, and ensure access to benefits and services per grant guidelines.

The role requires 2+ years in human services, supervisory skills, and strong communication. You will coordinate with PHC services and external agencies, maintaining accurate grant data and reporting.

Qualifications

• Bachelor’s degree or equivalent combination of education and experience (if allowed by granting agency).
• A minimum of 2 years of experience in human services.
• Supervisory skills with ability to provide direction and feedback.
• Excellent verbal and written communication skills with effective listening.
• Strong organization and time management with ability to work independently.
• Leadership skills to gather a team toward common goals.
• Proficiency with Microsoft Office and ability to learn new software.

Responsibilities

• Coordinates case management duties for assigned area(s), ensuring key tasks are completed.
• Leads team meetings for the grant team and attends all grant related activities.
• Provides initial consultation for escalated client situations.
• Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources.
• Monitors and documents client contacts, needs, goals, progress, and significant occurrences.
• Enters data into applicable data systems accurately and timely, following policies and procedures.
• Reviews data monthly to ensure accuracy and timeliness, coordinating with data system providers and grantors.
• Collaborates with Program Director and Grant Accountant to ensure grant spending is up to date.
• Facilitates coordination with PHC services and external agencies for referrals.
• Advocates for clients with community agencies, schools, and other agencies.
