Primary Health Care Inc. is seeking a Case Manager Team Lead for Homeless Support Services in Des Moines, IA. You will supervise staff, coordinate case management, and advocate for clients across PHC services and external agencies.

Responsibilities include intake assessments, data entry, and reporting to grantors. Qualifications include a bachelor’s degree, 2+ years in human services, and strong leadership and communication skills.

Qualifications

• Bachelor’s degree or equivalent combination of education and experience required.
• A minimum of 2 years in human services with supervisory experience.
• Strong verbal and written communication, and effective listening abilities.
• Organizational and time management skills with independence.
• Leadership experience with team coordination to goals.
• Proficiency in MS Office and internet-based apps; willing to learn new software.
• Ability to de-escalate situations and build relationships with partners.

Responsibilities

• Coordinates case management duties for assigned area, ensuring tasks are completed.
• Leads grant team meetings and related activities; provides initial consultations.
• Completes intakes and ongoing assessments; assists with entitlement applications.
• Monitors, documents, and reports client contacts and progress per guidelines.
• Enters data into systems; ensures accuracy and timely reporting.
• Collaborates with PHC services and external agencies for referrals.
• Advocates for clients with community agencies and schools.
