Job summary

Health Care for the Homeless Inc. is seeking an RN Case Manager in Baltimore, MD to provide nurse case management for housing services clients with behavioral health needs.

You will coordinate medical care, deliver nursing support, and collaborate with a multidisciplinary team to improve housing outcomes. You will perform nursing assessments, treatments, medication administration and adherence support, and conduct home visits, documenting in the EHR and developing SMART goals with clients.

Qualifications

• Bachelor’s Degree from an approved School of Nursing.
• Licensed in Maryland as a Registered Nurse.
• Two years of case management experience preferred.
• Two years of clinical nursing experience.
• Experience with behavioral health disorders preferred.
• Comfort working with homeless/low-income individuals.
• Ability to collaborate with cross-disciplinary teams.
• Willingness to apply Harm Reduction, Motivational Interviewing and Housing First.

Responsibilities

• Provide nurse case management services including medical care coordination and linkage to health resources.
• Deliver health education and counseling using client strengths and motivation.
• Perform nursing assessments, treatments, medication administration and adherence support.
• Conduct home and community visits and escorts per client plan of care.
• Document in the client’s EHR with required timeframes and clear notes.
• Involve clients in integrated treatment plans using SMART goals.
