Qualifications

• 5+ years of experience providing case management to vulnerable populations.
• Valid Driver's License.
• Use of personal vehicle.
• Education: High School Diploma and/or equivalent; bachelor's or associate degree preferred.

Responsibilities

• Screen clients upon admission and complete intake packets.
• Meet clients to assess needs and create a case plan.
• Coordinate with social/community agencies and refer as needed.
• Monitor progress, update treatment plan, and document outcomes.
• Educate clients and families on available resources and access points.
• Provide advocacy and crisis intervention when necessary.
• Attend community meetings and care conferences.
• Visit agencies, shelters, and hospitals to assess housing service eligibility.
