Job summary

Health Care for the Homeless in Baltimore, MD is seeking an RN Case Manager to provide medically-related case management within Housing Services. The role supports housing placements for individuals with behavioral health disorders, coordinating care with medical providers and the multidisciplinary team.

The position is full-time, onsite, with hours 8:00am-4:30pm. Salary range listed from $74,345 to $86,736, commensurate with experience.

Qualifications

• Bachelor’s Degree in Nursing required.
• Maryland RN license required.
• Personal vehicle and valid Maryland driver’s license preferred.

Responsibilities

• Provide nurse case management services incl. medical care coordination and linkage to health resources.
• Deliver health education and counseling addressing lifestyle choices and safety.
• Perform nursing assessments, treatments, medication administration and adherence support.
• Conduct home and community visits per care plan and escorts.
• Use client-centered, trauma-informed approach to care.
• Involve client in integrated treatment plan using SMART goals.
• Document in electronic health record per required formats and timelines (assessments, treatment plans, progress notes).
