Job summary

The Fortune Society is seeking a Supervising Case Manager to supervise Case Managers within the SRP, manage a small caseload, and conduct intake and assessments for SRP clients in New York. You will develop individualized service plans, provide counseling, ensure court appearances, and coordinate referrals to community-based resources.

Strong communication, organization, and culturally competent engagement with diverse participants are essential.

Qualifications

• High school diploma or GED/HSE required; BA in Social Work or related field desired; LMSW encouraged.
• Experience and commitment to working with criminal justice–involved participants.
• Ability to work with people from diverse backgrounds in a culturally competent manner.
• Strong Microsoft Office and Excel, and database entry skills.
• Strong written and verbal communication skills.
• Bilingual in Spanish a plus.

Responsibilities

• Conduct intake interviews and assessments for SRP clients.
• Perform general Case Management duties, including one-on-one check-ins and referrals.
• Develop client schedules and supervision plans addressing substance use, mental health, trauma, and housing.
• Assist clients in identifying and prioritizing needs and arranging voluntary referrals.
• Provide crisis intervention when needed in collaboration with supervisor.
• Enter client data accurately into the agency’s database.
