The Fortune Society is seeking a Supervising Case Manager to supervise Case Managers within the SRP, manage a small caseload, and conduct intake and assessments for SRP clients in New York. You will develop individualized service plans, provide counseling, ensure court appearances, and coordinate referrals to community-based resources.
Strong communication, organization, and culturally competent engagement with diverse participants are essential.
Supervised Release offers an alternative to jail, providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and non-violent felony offenses. Program participants are monitored to ensure their appearance at court dates, and they receive referrals to community-based programming. Under the Supervised Release program (SRP), the Supervising Case Manager (SCM) will supervise Case Managers and manage a small caseload, conduct intake and assessments for SRP clients, and develop individualized service plans to provide case management services; provide counseling, appointment reminders, and referrals (including setting up appointments for voluntary services and following up on service uptake); ensure individuals return to court; and enter real-time data on all client activities.
The SCM should align with the mission and culture of Fortune, including meeting clients where they are, engaging participants with a non-judgmental, inclusive attitude, holding space for trauma associated with criminal justice involvement and incarceration, and helping them believe in their power to change.
The SCM must communicate effectively with internal and external program staff and participants, and maintain communication to ensure clients are assisted in a timely and effective manner.
The SCM will receive a large number of referrals and must provide up-to-date and accurate information to Court Advocates and SCM Supervisors on client progress and participation. The SCM must maintain communication with internal staff and parties overseeing voluntary services. The role includes conducting client screenings and fielding calls from active clients and program staff. Prioritization is essential to manage the workload.
The SCM is the main point of contact for the client and often the most trusted; they are at the forefront of managing a client’s needs and must be able to problem solve when issues arise.
