Job summary

United Health Centers of the San Joaquin Valley is hiring a Lead Care Manager to serve as the primary contact for ECM-enrolled patients and their caregivers. The role develops comprehensive assessments and care plans with input from the patient, ensuring a whole-person approach to treatment gaps.

The position requires strong collaboration with the multidisciplinary team, ongoing outreach, and coordination of services, including hospital discharge planning and monthly provider contact.

Qualifications

• LVN license, MA certification or higher education; 3+ years experience preferred.
• High school Diploma or GED required.
• Valid driver’s license.

Responsibilities

• Serve as the primary point of contact for patient, family, AR, caregiver, and care team.
• Manage a full patient caseload per department guidelines.
• Conduct Comprehensive Assessment and develop Care Management Plan with input from patient and care team.
• Coordinate services to ensure seamless experience and avoid duplication.
• Maintain at least monthly contact with providers to coordinate care.
• Oversee ECM services and implement the care plan; follow up on referrals.
• Provide care where patient lives or seeks care; assist with discharge planning and hospital discharge coordination.
