Qualifications

• AA or higher in Drug/Alcohol, Social Work, Human Services, or related field.
• Experience coordinating services and care plans for ECM or similar programs.
• Strong communication and documentation skills.
• Bilingual Spanish/English is a bonus.

Responsibilities

• Coordinate with the member’s service providers and support network to ensure seamless ECM services.
• Actively engage ECM-eligible members and oversee care plan implementation.
• Connect ECM members to social services and transportation as needed.
• Advocate for members with healthcare professionals and stakeholders.
• Maintain HIPAA-compliant handling of PHI and documentation.
