Job summary

Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, navigate care options, and support engagement through field-based, virtual, or hybrid outreach.

You will partner with nephrologists and care teams to deliver timely education, coordinate referrals, and document interactions in Salesforce/EMR, helping patients move along the kidney care continuum with a patient-centric approach.

Qualifications

• High School diploma or GED; 3–5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.
• Experience with Salesforce/EMR systems is a plus.

Responsibilities

• Provide virtual care navigation for CKD and ESRD patients.
• Conduct scheduled and on-demand virtual or in-person patient interactions.
• Collaborate with nephrologists and care teams to coordinate patient care.
• Document patient interactions in Salesforce, EMR, or other platforms.
• Travel within the assigned market may be required.
